Leapfrog safety grades: Which Northeast Ohio hospitals earned A’s?

CLEVELAND, Ohio — Seven area hospitals — including four Cleveland Clinic and three University Hospitals medical centers – earned A grades in the Leapfrog Group’s fall 2025 Hospital Safety Grade. The Leapfrog Group, a nonprofit watchdog organization, assigns letter grades twice a year to hospitals, based on over 30 measures that track errors, accidents, injuries, infections, and preventive systems in place. UH said its Leapfrog grades highlight its focus on its Zero Harm initiatives that work to protect patients and deliver high-quality care. Ohio ranked 32nd, with 22.8% of its hospitals assigned A grades. Here is how Ohio ranked against contiguous states, listed from highest percentage of A grade hospitals to lowest.
