The 16-time European and three-time Olympic champion, who broke down in tears after winning bronze in the event at the Commonwealth Games last month, touched the wall in 59.10 seconds.
Reigning Olympic champion Nicolo Martinenghi took gold in 58.58 secs, with fellow Italian Simone Cerasuolo third in 58.90 secs, behind silver medallist Ivan Kozhakin (58.68 secs).
Briton Ramsay-Peaty, 31, finished just ahead of compatriot and Commonwealth silver medallist Filip Nowacki (59.11 secs), while Ireland’s Jack Kelly was seventh in 59.36 secs.
