According to the regulator, the incident occurred when the dozer was undertaking clean-up activities at an excavator face. It reversed and collided with a stationary rear dump truck that was queued at the loading area, damaging the dump truck’s emergency access ladder. The incident was found to be caused by inadequate positive communication between mobile plant operators, demonstrating the importance of effective communication, vehicle separation and operator awareness on roads and other work areas.
