The findings come as the Karnataka Food Safety and Drug Administration (KFSDA) stepped up inspections of hotels and restaurants in Bengaluru.
Dirty dining areas, kitchens or washrooms, and reuse of cooking oil were each reported by 29 per cent of respondents.
The survey question on overall standards received 2,235 responses.
Quarterly hygiene inspections and public disclosure of restaurants violating food-safety norms were each backed by 76 per cent of respondents.
