New Castle - Jeffrey D. Flak, 59, formerly of Almira Avenue, died Saturday, Aug. 8, 2026, at Quality Life Services in New Castle. He was born Sept. 20, 1966, in New Castle, a son of the late Gordy and Marcyne (Biddle) Flak. In addition to his parents, he was preceded in death by one daughter, Jessica Flak; and one sister, Christine Flak. Visitation will be 4 to 6 p.m. Wednesday, Aug. 12, 2026, at the William F. & Roger M. DeCarbo Funeral Home. A funeral service will be celebrated at 6 p.m. Wednesday, Aug. 12, 2026, at the William F. DeCarbo Memorial Chapel.
