Blackburn man died after insulin prescription error between hospital and nursing home

Sylwester Wasilewski, who suffered from type 1 diabetes, died on December 18 at Hawkhurst Nursing Home from the effects of hypoglycaemia, a condition where blood sugar levels drop below 70mg/dL.
An inquest into his death heard that Mr Wasilewski, 54, had been an inpatient in Royal Blackburn Hospital between October 13 and his discharge on December 17.
Controlling his diabetes with insulin, Mr Wasilewski had been recommended to reduce his short-acting insulin dose, but his prescription was not changed.
“Mr Wasilewski was found unresponsive a short time after, and despite resuscitation he was pronounced deceased by paramedics.
“Mr Wasilewski has a number of underlying comorbidities which contributed to his death.”
