Platform Housing Group’s Falls Response team, which is funded by the NHS, is a finalist in the NHS Team of the Year category at the Worcestershire Health and Social Care Awards 2026. As well as dealing with falls, staff frequently identify wider health and wellbeing issues during visits, making referrals to other professionals and support services so people can remain safe and independent at home. In 2024 alone, the team made 271 onward referrals to occupational therapists, GPs, community nursing teams, social care services and charities such as Age UK. Falls Response service manager Lauren Adam, who nominated the team, said: “Every member of our team genuinely cares about the people we support and always go the extra mile. The Worcestershire Health and Social Care Awards celebrate outstanding contributions to health and care across the county, with winners due to be announced later this year.
