James Walker, from Stockton, has since experienced ongoing health problems including irregular heart rhythms, breathlessness, and fluid retention.
Since his heart attack, the 63-year-old has been supported by the heart failure hospital at home team, part of the cardiac remote monitoring service, who have cared for him at home with regular visits and remote monitoring technology.
He said: "Even while I've been back in hospital this time, members of the heart failure service have popped their heads through the door to see how I am.
Trudy Hunt, a heart failure specialist nurse, said: "Feedback from patients like James means so much to us all – we are really proud of how far the service has come.
To help improve the service, the team is currently running a survey for patients and their families.
