"While it is clear that the medication error occurred, the available evidence does not support a finding that the medication error caused or contributed to the natural disease processes implicated in Mr Wolstenholme's death, namely gastrointestinal bleeding, pneumonia, and kidney failure," Coroner Spanos said.
