Investigation ongoing more than eight months after fatal workplace incident

Žavinta Petrašiūtė, who lived in Wisbech, died at Pretoria Energy Mepal on November 17, 2025, after she was struck by a digger.
Recording a provisional cause of death, Coroner Elizabeth Gray revealed Ms Petrašiūtė had suffered "multiple traumatic injuries".
In an update on Wednesday (August 5), the Health and Safety Executive (HSE) confirmed an investigation into the incident remains ongoing.
Pretoria Energy Mepal, which is based between Mepal and Chatteris, was issued with an improvement notice by the HSE on November 20.
It was found to have breached the Health and Safety at Work Act 1974 and the Workplace (Health, Safety and Welfare) Regulations.
