A Melbourne grandmother has been left permanently disfigured after hospital staff mixed her up with another patient with the same first name. A catastrophic hospital mistake left Melbourne grandmother Debra Buchanan permanently disfigured after staff mistook her for another patient with the same first name. Debra Buchanan woke up after a routine procedure and haemorrhaged everywhere before realising her skin cancer had not been removed. She then had to return to the same hospital to have the skin cancer removed. Debra Buchanan said she was terrified to return to the same hospital to have her skin cancer removed after the wrong surgery was performed on her.
