PC 2563 Stuart Ayling is due to appear before a misconduct panel at Cumbria Police headquarters in Penrith on Monday, August 10, and Tuesday, August 11.
The officer is accused of breaching professional standards relating to duties and responsibilities, discreditable conduct, and honesty and integrity.
The third allegation concerns entries allegedly made by the officer on what the force describes as a 'minimum standards checklist'.
The hearing is scheduled to begin at 9am on both days at Carleton Hall, Cumbria Police's headquarters in Penrith.
Anyone wishing to attend must apply to the force's Professional Standards Department before 5pm on Friday, August 7, and will be required to provide accurate personal information and formal identification.
