OSHA does not find Jacob’s Pillow at fault for last year’s deadly accident that killed an employee

Becket — One year after an accident that took the life of Jacob’s Pillow Production Manager Kat Sirico, the Occupational Safety and Health Administration (OSHA) issued an inspection report and has determined that the organization is not at fault.
The report was issued Monday, August 3, and The Berkshire Edge obtained it after making several requests to OSHA over the past year.
On August 1, 2025, Sirico was killed in an accident that occurred outdoors on the campus.
Days after the accident, the organization cancelled the rest of its performances for the season.
OSHA officials interviewed several employees of Jacob’s Pillow for its report, and officials recommended that no citations be issued to the organization for the accident.
