Police are requesting information on Daiyanaliz Diaz, of Chelsea, who had been in DCF custody at Newton Wellesley Hospital before leaving without authorization July 3. Police describe Diaz as a dark-skinned Hispanic female, 5 foot 3 inches tall, petite build, pulled-back black hair and brown eyes.
