According to the regulator, the incident occurred when a loaded haul truck arrived at the run of mine bin to unload at the end of a shift, with a separate truck already tipping at the time. The driver of the stationary truck was still in the cab and attempted to warn the reversing haul truck operator by sounding the horn before exiting the cab, but the reversing vehicle collided with the parked unit.
