After that fatality, the Rail Accident Investigations Branch (RAIB) issued safety recommendations in May 2017 to all rail companies, including GWR. The ORR wrote to GWR to the rail company to ‘highlight the shortcomings’ of that assessment it had carried out in 2017, telling the rail company it was ‘neither suitable nor sufficient’. It took Bethan’s death to force GWR and other train companies to finally act, and droplight windows were either phased out or made safe on all trains. “Her death was a preventable tragedy that highlights the need for train operators to proactively manage risks and act swiftly when safety recommendations are made to keep their passengers safe.
