Some Greater Minnesota providers say the state’s fraud crackdown has left legitimate caregivers caught in a revalidation process they say was delayed and unclear, with the vast majority of terminated providers disenrolled over paperwork problems.
More than 2,700 providers have appealed their termination as the Minnesota Department of Human Services (DHS) faces questions about its revalidation process.
As she explains, DHS has been routinely revalidating providers long before fraud concerns forced the state agency to revalidate thousands at once.
When asked whether she felt that was proper notice by DHS, Fairchild said, “No, and so it was disheartening.”
“As of July 23, 2026, 2,732 disenrolled providers have appealed their disenrollment; 748 of these providers have had been successfully revalidated.”
