Find out more about the winner and shortlisted entries in the Nursing in the Community category at the 2025 Nursing Times Awards. Recognising this quality improvement gap, we initiated a project to empower community nursing teams to deliver safe, effective TWOCs using evidence-based tools. We did this by advocating through educating clinicians in both the community health sectors and hospital to increase understanding of homeless health. The main challenge was recognising and appreciating the differences in community service delivery and the differences in which people are seen and treated.
