The Care Quality Commission (CQC) inspected Leicester City Council's Shared Lives service in a routine visit earlier in June 2026.
This meant that the city council service was "relying on outdated information which did not accurately reflect the person's current risks or the support required to keep them safe".
A Leicester City Council spokesperson said: "We accept the findings of this inspection and are committed to making the improvements identified.
