Case 1A 45-year-old man presents with monomorphic 1-mm vesicles on an erythematous base on the lateral left thigh distributed linearly along the L5 dermatome. Herpes simplex virus infection lesions can have indistinguishable morphology from VZV infections: 1- to 2-mm clustered vesicles on a brightly erythematous base. Lichen planus is usually intensely pruritic, unlike the mildly pruritic lichen striatus and generally asymptomatic blaschkitis. Blaschkitis typically consists of papules and plaques, and older lesions are often scaly. Lichen striatus and blaschkitis are both expected to self-resolve; however, blaschkitis can often demonstrate relapses.
