Rail review as it happened: NSW Government releases scathing report into Sydney trains

The report said while repairs to the wire “were efficient and a credit to the team involved” once the affected train was removed, it took too long to get the crew on site.
A “Track Occupancy Authority”, which allows repair vehicles onto the tracks, was requested at 10.25pm and authorised at 10.55pm.
At 12.29am, the authorisation was reissued and repair vehicles were moved into place 16 minutes later, before they were soon ordered to be removed again.
An hour later, another plan was rejected and a final successful approval was put in place by 2.52am.
“Lengthy delays in completing the repairs meant that significant disruptions occurred for the rest of the day,” the report said.
