"Significant concerns" were raised about inadequate adult support and protection measures, particularly around sexual safety, as well as issues with governance, patient safety and staff training. "Improvements are required in the oversight of patient and staff safety incidents, including delays in reviews of adverse events. Eddie Docherty, director of quality assurance and regulation at HIS, added: "Our previous mental health inspection programme was focused on infection prevention and control. This included changes to improve patient safety and reduce potential risks, both within the Unit and its surrounding external areas. “Training compliance has improved in the acute mental health unit over the last six months since the inspection took place and we have also successfully recruited more than 30 mental health nursing graduates to support mental health services across the organisation.
