A “failed” safety check led to Lee Hulse’s death on the Valaris 121 drilling rig in the North last year, the Health and Safety Executive (HSE) concluded. Reports from those onboard the Valaris 121 at the time claimed that Hulse fell through a window after a seal was not put back in place following cleaning.
