He subsequently spent multiple weeks receiving treatment at a mental health facility in Bury, reports Lancs Live. At an inquest into Simon's death, held on Wednesday January 28 at Preston Coroners' Court, his wife Sarah said: "I believe there was a failure in aftercare from the NHS." ‌Both Sarah and Simon contacted LSCFT's Initial Response Service and Home Treatment Team on multiple occasions during the month preceding his death.
